Intensive Case Management for Seniors at Risk
Intensive Case Management for Seniors at Risk (ICM) provides intensive case coordination with an in-depth, personalized, flexible, and creative approach. This program specializes in elder abuse, including physical, psychological/verbal, financial, neglect and/or self-neglect. Anyone may call to make a referral. The service is free and confidential.
Overview of Services

Case coordinators offer home visits, where a comprehensive, holistic assessment of the client’s situation is completed; then relevant, realistic, and attainable options are discussed

Consultation offered via telephone for families/caregivers who do not wish to receive a home visit

The program is client driven and client-centered

Cultural differences and personal preferences are always taken into consideration to preserve autonomy
Primary goals:

Ultimate goal is to improve the client’s quality of life by utilizing the least intrusive and most accommodating approach

Client criteria:

60 years of age and over
